Feature Request: Customizable Block Page, Request Form, and Messages

It would be great to have more customization options for the block page and the request submission flow.

Currently, the block page displays information such as the block rule that triggered it, along with a request form asking for a name and message. After submitting a request, it displays a confirmation message such as “Thank you for your submission!”, while the block rule information (e.g. “Blocked by Custom Rule (www.domain.com)" is shown in a styled font/red box.

It would be useful to have settings that allow users to customize or disable these elements individually:

Block rule information — Option to hide/remove it entirely or customize the text and appearance.
Request form — Ability to customize the form fields, including changing, removing, or adding fields such as Name and Message.
Confirmation message — Ability to customize the message shown after a request is submitted instead of being limited to “Thank you for your submission!”
Block page error messages — The error message can already be customized, but it would be useful to have a variable or placeholder that automatically inserts the specific address that was blocked. For example, allowing a custom message such as “Access to [blocked domain] has been blocked due to network policies.” This would make the custom message more informative while still allowing users to control the wording. It would also be helpful to increase or remove the 255-character limit, allowing users to create longer and more detailed custom messages when needed.
Block page appearance — Ideally, allow customization of the text, colors, formatting, and visibility of these elements.

For example, instead of being forced to display “Blocked by Custom Rule (www.domain.com)" in a red box, users could replace it with their own message or choose not to display the rule information at all.

Overall, it would be nice to have a fully customizable request/block page, giving users control over the request form, block rule information, confirmation message, error messages, and their appearance.
